| As part
of the Primary Care Loan Service Requirement,
recipients of the PCL loan (and the EFN/FADHPS
scholarships) are required to report their primary
care internship/residency and practice status to
OU-HCOM on an annual basis until the loan is repaid in
full. Below are the on-line forms that allow
graduates to fulfill this requirement. Please
complete the form that matches your status. The
forms should be completed by July 1st of each year.
