Holzer Hospice

it blank and hit enter) is a licensed, certified hospice program
that provides services in the home and in contracted nursing
home agencies, to patients with life-limiting illness, and
their families. The program provides medical symptom
management by the patient's attending physician in
conjunction with an interdisciplinary team of skilled
practitioners and volunteers. Service area includes Gallia,
Jackson, Meigs and portions of Vinton and Lawrence Counties

Special Notes:
Please
call the Holzer Hospice office the day before your CCE to
learn your final destination - this will make it possible
for you to drive directly to meet the Hospice nurse and not
first to Gallipolis.
You are REQUIRED to call
the office on Friday if you
have a Monday CCE or on Monday if you
have a Thursday CCE to CONFIRM
YOUR ATTENDANCE. If not, the CCE will be canceled and
considered an
unexcused absence. If you get the answering machine when you call,
leave your name and number and reason for the call.
If you
will be arriving even a few minutes late, please call the
agency contact person to explain. There may be a nurse
waiting for you to go on a home visit.

Specific CCE Goals:
· Understand the hospice philosophy of caring
for the dying person.
· Observe the emotional, physical and familial
dynamics of a terminal illness.
Objectives:
By participating in this CCE
assignment, medical students will:
· Review literature on hospice and identify the
roles of the hospice team, including services provided,
costs and client stipulations.
· Identify the stages of death and dying
typically manifested in persons with terminal illnesses.
· Explain the use of medications, side effects
and dosages needed to relieve and manage pain.
Equipment Needed for Activity:
Stethoscope
Dress:
White lab jacket and
identification badge. Please wear professional casual
attire, no blue jeans or tennis shoes.
Transportation:
You are required to drive to the
agency, as you may need to drive your own car to home
visits.
Responsibilities of the Medical
Student:
· The patients you are visiting may be immuno-compromised,
so if you have a communicable disease please reschedule your appointment.
· While on agency property, medical students
will conform to all applicable policies and procedures of
the agency.
· Medical students will exercise confidentiality
with regard to all patient and/or staff information gained
during the clinical experience.
· Medical students are financially responsible
for any illnesses, injuries, or other health problems they
may incur during their clinical experience.
Directions:
Take 33 East to Pomeroy (the new highway). Take
the SR 7S exit toward Gallipolis. Take SR 35 west, go
about 5 miles and take the Gallipolis exit (Route 160) to
the right. At the intersection, look for the brown and
tan building on the left. Turn left at the traffic
light; the building you want is the first one you see. The sign says "Hospice,
Extra Care and Tobacco Prevention Center" and the
building has #2281 in large numbers on the front.
