Community Health Programs (CHP) provides
free or low cost services to residents of
Southeastern Ohio. We are committed to our
community and work to bring quality
healthcare to those who have the greatest
need. Each of our many programs is
coordinated by our highly qualified and
dedicated staff. We are continually
developing new programs to better meet the
needs in Appalachia. We encourage you to
contact us with ways that we can better

Free Clinic
The mobile van travels two to three times a
month to
Stewart, Glouster, Albany, Pomeroy,
or Nelsonville, Ohio (map),
but also has a location on the 2nd floor of
Parks Hall in Athens, where the Diabetes
Clinic also meets once a month. For further

Diabetic
Clinic
The Diabetic
Clinic started in 2006 and sees patients
every first Tuesday of the month in Parks
Hall, 2nd Floor, in Athens, Ohio. There are
no travel diabetic clinics as of this date.
This clinic is open to the uninsured and
underinsured and has access to the Patient
Assistant program, which offers free
medications to individuals who qualify. |
