All Applicants: See individual programs for due dates

International Programs Application/Conditions of Participation - application form for all OUCOM international programs.

HTH Worldwide Health Insurance Enrollment

Visa - links to the U.S. Department of State and lists the requirements for entry to the country you are traveling to. There are also links to the appropriate embassy websites.

Embassy registration - links to the U.S. Department of State website in order for you to register. Please remember to print or copy your information before you click submit and send a copy to Mary.

Confirmation of Embassy Registration

OU Waiver Form - for travel to country with US State Department travel warning, Currently, this includes Kenya.

Non-Degree/Preceptors/Miscellaneous Forms

Non-Degree Application - required from each non-OU student or employee. Nonmedical participants:  Send copy of transcripts or diploma. Licensed health care providers:  A photocopy of your state medical certification/license.  The application fee is waived if paperwork is sent through Mary Dailey.

International Programs Application (Preceptor) - application form for preceptors who will be participating in an OUCOM international programs and precepting OUCOM students.

Preceptor Information Form (Clinical) - volunteer faculty information form for international clinical rotations.

Preceptor Information Form (Research) - volunteer faculty information form for international research rotations.

Supplemental Application - application form required only for independent rotations.

International Student / Teacher Identification Card - Ohio University no longer requires this, but you may want to get one because you can get discounts in some countries.
