APPLICATION PROCESS FORMS

All Applicants including preceptors:   Please complete the on-line application process a
t 
http://eais.admsrv.ohio.edu/eais/student. See the individual program sites for additional requirements and application due dates.

Supplemental Application for International Rotation - application form required only for independent rotations.

Non-Degree Application - required for all non-Ohio University students, volunteers, advisors or employees.
*The application fee is waived if paperwork is sent directly to the Office of Global Health Programs.

If you do not already have an OHIO e-mail address, you will also need to activate one at
https://www.ohio.edu/myid You will need the PID Number and date of birth supplied to you in the letter from Admissions after submission of the Non-Degree Application.    

Nonmedical participants:  Send copy of transcripts or diploma.

Licensed health care providers:  Provide a photocopy of your state medical certification/license.

Embassy registration - links to the U.S. Department of State website in order for you to register. Please remember to print or copy your information before you click submit and send a copy the Office of Global Health Programs.
