Non-Degree/Preceptors/Miscellaneous Forms
Non-Degree Application
- required from each non-OU
student or employee. Nonmedical participants: Send
copy of transcripts or diploma.
Licensed health care providers: A photocopy of your
state medical certification/license.
The
application fee is waived if paperwork is sent
through Mary Dailey.
International Programs
Application (Preceptor) - application form for
preceptors who will be participating in an OUCOM
international programs and precepting OUCOM students.

University no longer requires this, but you may want
to get one because you can get discounts in some
countries.
