Supplemental Application
- application form required only for independent
rotations
Conditions of Participation in
an International Program
- needs to be completed by each participant
Non-Degree Application
- required from each non-OU
student or employee. Original transcripts are
required for students; a copy of your state medical
certificate/license will be accepted in lieu of
transcripts for licensed health care providers. The
application fee is waived if paperwork is sent
through Mary.
International Student /
Teacher Identification Card - everyone needs an
international ID card. Be sure to include a passport
quality photo to place on the card. This card also
provides emergency evacuation insurance. You may be able
to obtain an international identification card
from another university.

Visa - links to the

for entry to the country you are traveling to. There
