International Programs
Application - application form for all
OU-HCOM international programs
International Programs
Application (Preceptor) - application form for
preceptors who will be participating in an OU-HCOM
international programs and precepting OU-HCOM students
Conditions of Participation in
an International Program - needs to be completed by each participant
International Student /
Teacher Identification Card - everyone needs an
international ID card. Be sure to include a passport
quality photo to place on the card. This card also
includes your evacuation insurance. You may be able
to obtain an international identification card
through your own university.
Non-Degree Application
- needs to be completed by each non-OU
student or employee. Original transcripts are
required for students; a copy of your state medical
certificate/license will be accepted in lieu of
transcripts for licensed health care providers. The
application fee is waived if paperwork is sent
through Cheri.
Embassy registration
- links to the U.S. Department of State website in
order for you to register. Please remember to print
or copy your information before you click submit and
send a copy to
Visa - links to the
U.S. Department of State and lists the requirements
for entry to the country you are traveling to. There
are also links to the appropriate embassy websites.
Evaluation - evaluation form
for your program, to be completed after you finish
the program.

Please complete this form if you agree to the
policies outlined.
Preceptor Information Form -
Please complete this form if you agree to supervise
students in the clinic. The program will not
cover the cost of your airfare without this form.
Recommended Reading List - It
is imperative that you prepare for your clinical
experience. You will be expected to give a
presentation on a specific topic while in Kenya.
Topics will be assigned at orientation.

for your trip.
