The OU-COM identification system will provide a means to instantly identify the individual and educational level of an OU-COM medical student and/or physician in postgraduate training in both the clinical and academic setting.

  • A color-coded identification (I.D.) badge with photograph will be issued to each OU-HCOM medical student/postgraduate physician by the responsible department at the appropriate time as follows:

  • Each I.D. badge for OU-HCOM students, interns, residents, and visiting students will indicate the following:

            Ohio University Heritage College of Osteopathic Medicine

            College logo

            Year of medical education

            Regional Teaching Center if 3rd year, 4th year, Fellow   

            Fellowship Program, if applicable

            Level of Postgraduate Training, if applicable

            Affiliated Hospital if Intern/Resident

            Expiration Date 

  • If a student’s personal appearance changes drastically between Year 3 and the start of the 4th year, a new photograph will be taken upon the student’s request and a new I.D. badge processed.  However, a replacement fee will be charged.

  • The OU-HCOM Photo I.D. badge will not supersede the Ohio University I.D. card issued for University Services.

  • The OU-HCOM Photo I.D. badge must be visibly worn at all times (preferably on the lapel of white lab coat) while in the clinical setting.  This includes the Clinical Training and Assessment Center as required in lab situations by course coordinators.

  • The OU-HCOM Photo I.D. badge must be visibly worn at all times while taking an OU-HCOM exam unless informed otherwise by the course coordinator.

  • The OU-HCOM Photo I.D. badge must be presented upon request while using the Heritage College of Osteopathic Medicine facilities.  When at the Learning Center, the OU-HCOM Photo I.D. badge must be presented in order to check out books. Either the OU-HCOM I.D. or the Ohio University I.D. must be presented in order to use certain restricted materials.

  • Loss or damage to the OU-HCOM Photo I.D. badge must be reported to either

             a) Student Affairs (Year 1-2 students) or

             b) Academic Affairs (Years 3-4 students, interns, residents, and any visiting students)

  • If a student/postgraduate physician changes his/her last name, issuance of a new OU-HCOM I.D. badge will be required.

  • A $5.00 replacement fee will be charged to have a new OU-HCOM Photo I.D. badge made.

  • 3rd year and 4th year students who are required to wear a hospital I.D. card at their regional site must also visibly wear the OU-HCOM Photo I.D. badge.

  • The OU-HCOM Photo I.D. badge should not be defaced in any way.

  • Duplicate Photo I.D. badges will be identified as such.  If the original I.D. badge is found or returned, the duplicate I.D. badge must be returned at once to Student Affairs.

STUDENTS WILL BE REQUIRED TO PERSONALLY

 SIGN FOR THE RECEIPT OF THE OU-HCOM PHOTO I.D. BADGE.
