Purpose
The OU-COM identification system for faculty, staff and student
employees provides a means for students, patients, visitors and staff
members to quickly and easily identify OU-COM employees and their
department/unit affiliation. The identification system will also be used
for security access.
Medical students need to refer to the Student Handbook for
policies concerning identification systems for OU-COM students.
Policy
-
OU-COM Human Resources will issue all faculty, staff and student
employees an identification (ID) badge that includes a photograph of
the employee and the following information:

name and academic credentials
·
college name and department or unit affiliation

The OU-COM ID
badge is issued in addition to the Ohio University ID card, which is
issued for the purpose of access to many university services.

All employees on the OU-COM Athens campus are required to wear the
OU-COM ID badge so that the photograph and other identifying
information are easily viewed. The badge should be worn in an easily
visible location at all times while on the OU-COM campus.

Employees are responsible for having a new ID badge made if:

their personal appearance has changed substantially
·
a name change has occurred
·
the badge is damaged, or
·
the badge is lost or stolen.

Replacement costs for defaced, lost or stolen badges are the
responsibility of the individual employee unless otherwise specified
within this policy. When either the OU-COM ID badge or the Ohio
University ID card is lost, both need to be replaced at the same
time. To obtain a replacement for an OU-COM ID badge and the Ohio
University ID card, the employee should visit the
Technology Depot in Baker University
Center 112.

Lost badges
should be reported immediately to the Ohio University Police
Department and the OU-COM Human Resources office. They will
immediately deactivate the lost badge.

The ID badge is OU-COM property. Defacing the ID badge is prohibited
(i.e., stickers, pins, etc.). However, stickers or pins attached to
badge holders are permitted, but must not obstruct the employee
photograph or other identifying information on the badge.

Unauthorized possession, use or reproduction of an OU-COM ID badge
may constitute theft or misappropriation of OU-COM property and will
be grounds for disciplinary action up to and including termination
of employment or prosecution by law.
