The book sales will be held from 12-2
p.m. on Thursday August 7th, Friday August 8th,
Tuesday September 9th, and Wednesday September
10th.
There will also be a “mini-book sale”
of only Gross Anatomy Books held during Orientation
(Friday August 1st) from 11-2 p.m. on the bricks
in Irvine Hall.

Instructions:

Fill out your name,
address (WHERE the check is to be sent) and e-mail.
Please note: checks will be mailed in
fall, so make sure to write your NEW address
on this invoice and keep us informed of address changes via the
e-mail listed below.

Complete the
following form with title, edition, author & asking price for
each book you want to sell. (Also, please write your last
name on the inside of the book cover or make sure to group your
books together somehow before bringing them to the drop-off
date.)

Accept the
Disclaimer Conditions at the bottom of the form.

Print out the
"Confirmation Page" you get after you hit the "Submit" button
below.

Once you have
printed the “Confirmation Page” please MAKE SURE TO PRINT YOUR
PID/SSN in space provided. THIS IS
MANDATORY FOR REIMBURSEMENT! (OU requires this now for tax
purposes.)

Bring your books AND
the "Confirmation Page" to one of the "Drop-Off Days" in Irvine
Hall:
June 6, 11-2 pm (during 4th year exit counseling);
July 24, 12-2 pm; and
August 15, 12-2 pm
If you cannot make one of the times listed above, please contact
the FPSA officers at:

to ship your books to one of the officers.
