2. Complete administrative items

c    Complete your back-ground check Monday, June 22 and Tuesday, June 23 at Scott Quad.  *See published schedule and section list for your appointed time.              

      Students must bring their current drivers license or state issued I.D. to the OUPD Office at the time of the background check. The criminal background check will cost $58.00.  Cash, personal check, money order, bank check or certified bank check for $58.00, made out to Ohio University, will be required at the time of the background check.   The $58 is required so that the fee and fingerprints can be processed at the Bureau of Criminal Investigation and Identification (BCI&I) for facilitation of the BCI & I and FBI criminal background check. A form will need to be completed while at OUPD. Please give a copy of your receipt to the Admissions Office Staff member upon completion of your background check. We will make a copy of the receipt for our files and the original receipt will be returned to you.  Background checks will be conducted at the Ohio University Police Department in Scott Quad on the OU campus. A detailed map showing Scott Quad can be found at http://ohiou.edu/athens/ioumap.html. A detailed schedule of when you are to report for your background check will be placed in your mailbox and emailed to you in mid-June. Do not deviate from the assigned schedule.  Any questions regarding the background checks can be directed to Dr. John Schriner or Sarah Tipton in the Admissions Office.                                                  

                                               

c    Obtain a screening for TB, either by skin testing in the Parks Hall Immunization Clinic scheduled the week of June 22 or by making an appointment for a verbal review if you have a history of positive skin tests.  Make sure your immunizations and antibody titers are up to date. Contact Sarah McGrew or 593-2518 for details.              

c    Pay any delinquent bills in the Bursar's Office in Chubb Hall. Here's info on paying your bill.

c    Check the LRC for any copying bills that need to be paid.  Return all LRC and Alden Library materials.

c    If you have the Ohio University Student Health Insurance, remember, every year you need to get a NEW insurance card, so be sure to print one out!  More details to follow on June 24, 2009.

c    Complete your web-based Bio Sheet Form by Friday, July 17, 2009.

c    Familiarize yourself with the Year 3 and 4 manual for Class of 2010.   

 

3. Check-out of Grosvenor        

c    Empty your Grosvenor mailbox by noon on Friday, July 17.  

c    Turn in all building keys to Student Affairs by Friday, July 17. There is a $10 charge for every key not returned. 

c    Clean out OMM & Gross Anatomy lockers in Grosvenor by Friday, July 17. Otherwise custodial services will dispose of all contents.   

c    Strongly advised (though not mandatory): Purchase an extra white coat at Follett’s University Bookstore (740-593-5547):  $21.00, no patch.

Academic Record: How can I get a copy of my transcript?

IMPORTANT --  When ordering academic transcripts from the Registrar's Office for post-graduate training applications: Before you order a transcript to be sent to a training institution, it would be wise to order a copy for your personal review. This will allow you to verify that the transcript correctly reflects your progress through the curriculum. It will also give you a current picture of your grades that have been completed in the system. You can order a transcript on the web at the following site: http://www.ohiou.edu/registrar/transcri.htm

You can also order specific quarter grade reports through the world wide web. Through the site http://www.ohiou.edu/registrar/grades.htm you view your grades online or you can receive an individual quarter report through e-mail within 24 hours of the request.

Board exams:

i) When do I take osteopathic national boards?

Beginning with the graduating class of 2008, all students must pass COMLEX Levels 1 & 2 CE and COMLEX Level 2 PE in order to graduate.  Level 2 of the National Boards will be offered each year as indicated at the NBOME website: http://www.nbome.org/ .  The exams are on computer and the application process is on-line. Please check the NBOME website for test dates, test sites, costs and the application process.

In order to be fully prepared for COMLEX Level 2 CE, OU-COM requires students to successfully complete the following rotations prior to taking the exam:

OCOM 862 General Surgery (4 weeks)

ii) How do I get my board scores sent to an institution?

Student Affairs handles all requests for board scores.  If you need board scores sent at the time you are requesting a letter of recommendation, just check the appropriate box on the Letter of Recommendation Request Form.  If you require additional board scores sent at a later date, there is a separate form for requesting that service.  The copies of board scores will be sent at no charge, but we must have a completed request.  If there is a reason you do not want a particular set of board scores sent, you must inform the Student Affairs office of the special request.

Once again, the written request must be completed thoroughly to insure proper and efficient handling.  Please allow at least two weeks to process requests.

The request must be typed or clearly printed with the complete address of the hospital/institution and accompanied by the student's signature.  Send to:  740-593-2399, via email to Molly or mail to:

Graduation & Beyond:

i) How do I receive materials so I can graduate?

In addition, address update requests will be sent to you so that Student Affairs can maintain communication during your last six months as a student.

REQUIREMENTS FOR GRADUATION:

1.  Only those candidates for the degree of Doctor of Osteopathic Medicine who have completed all of the requirements for such a degree will be allowed to participate in the official OU-COM commencement ceremonies.

a) Exceptions to the above policy may be granted by the Dean on an individual basis after his/her review of a specific request from a candidate that details the reasons why such an exemption should be granted.

b) The Dean's decision to grant an exception to the above policy will be guided by reasonable estimate of the likelihood that a candidate will be able to complete the requirements for the awarding of a D.O. degree within 60 days of the official date of graduation.  An example of such a circumstance might be a student who had to miss two weeks of a required rotation because of illness or emergency and scheduling logistics prevented making up that time prior to graduation.  Such a candidate could reasonably be expected to be able to complete the requirements within 60 days following commencement and therefore should be allowed to participate in the ceremonies.

Scheduling: How are rotations coordinated? 

You must work with your CORE Administrator to schedule all rotations – even those outside the CORE.  Your CORE Administrator is your first point of contact for all scheduling and will provide all the correspondence necessary for your "outside" rotations.  Your CORE Administrator has all the answers when it comes to your clinical rotations.

 

Communication:  How will it happen?

There are two main ways that communication takes place among CORE sites and the OUCOM Athens Campus. 

1.) Ohio University (OAK e-mail) account.  Do NOT forward your e-mail to personal accounts (Hotmail, Yahoo, Gmail, etc), as some sites are refusing to guarantee delivery to those.  

*You can turn off your forwarding on the web by going to: www.cns.ohiou.edu/email.  Refer to that website for additional information on using your OU e-mail account. 

**To see what your email address or Oak ID is, simply look yourself up in the University's on-line directory. Your Oak ID and email address will be listed.

2.) Your mailbox at your CORE site.

YOU are responsible for checking BOTH your email and mailbox frequently, as timely information will be sent to you.  If you have trouble with your e-mail account or are not receiving mail at your CORE site mailbox, please contact Student Affairs immediately.

i) What if my name, marital status or address changes after I leave campus?

Follow the procedures on How to Process a Name Change and please update your Bio sheet as instructed above.  If you have technical problems please call 593-0056. If you have RAC code or password concerns contact David Wolf at 593-2297.

Up-to-date addresses are used by the College and University for periodic mailings, transcripts, financial aid refunds, and other pertinent materials.  If you do not notify us of name and address changes, your record will be incorrect, and this could cause problems upon graduation.  Please send all requests and other information to:

Financial Aid Issues:

i) How will I get my money?

If you do not have direct deposit, your refund checks will be mailed by the university to the address that you have on record in OU-COM’s Student Affairs office.  Please be sure to update this address if you move or do long out-rotations while you are at your CORE site.

Now that you are leaving Athens, this is a great time to consider signing up for Direct Deposit of your refund check.  Follow this link : http://www.finance.ohiou.edu/disbursement/directdeposit.html

ii) When will I receive my overage checks?

You will find these dates on your award letter which is forthcoming. 

iii) If my current academic year runs from June 2009 until June 2010, why is my fall quarter overage check smaller than the overage check I received for summer quarter?

Ohio University’s annual tuition increase is effective beginning fall quarter, so a higher amount of tuition and fees are being applied against your fall quarter loan disbursement. 

iv) What if I run out of money and need an Emergency Loan?

An E-version of the Emergency loan form can be found on our web site at:

http://www.oucom.ohiou.edu/Aid/fin_aid_emergency_loan.htm. It takes about 48 hrs. to process an emergency loan. 

v) How will I know what is going on with my Financial Aid?

Now that you will not be on campus, it is more important than ever to make it a habit to regularly check for messages on your OU e-mail account.  The university uses e-mail to alert you when your financial aid has arrived.  In addition, Student Affairs uses e-mail to send out important announcements that you won’t want to miss.

Insurance: Am I still covered by Ohio University’s plan?

Yes.  For details, attend the Wednesday, June 24, session with Brenda Dorst, Hudson Health Center Representative from 12:00 noon to 1:00 pm in Irvine Hall 194.  Also visit the Hudson Health Center website for details at www.ohiou.edu/hudson and select Student Health Service.

 

Student Government: Who represents me at my CORE site?  

Each CORE site has an elected representative from their respective classes.  Their job is to coordinate communication with Student Government and manage funds available at each CORE site.  As the Student Government advisor, Holly Jacobs can help in answering questions, financial and other, but you should check with your CORE site representative and your CORE site administrator first.  Also, please feel free to e-mail Student Government President, Lauren Fuller.
