Student Identification System

 
   
Purpose

The OU-HCOM identification system provides a way to establish the identity and educational level of an OU-HCOM student or physician in the graduate medical education (GME) programs when in clinical and academic settings.

Protocol

A color-coded identification (I.D.) badge with a Polaroid photograph will be issued to each OU-HCOM student/postgraduate physician by the responsible unit (i.e., the Office of Student Affairs = SA; Academic Affairs-Regional Education Programs = AA/REP; Southeast Center for Osteopathic Regional Education = SE CORE; and the Director of Medical Education Office = DME) at a specified time as follows:

* At the same time students have pictures taken for their Year III Photo I.D. Badge, an additional picture will be taken for their Year IV Photo I.D. Badge. This will save time, personnel, and cost.

** Pictures will be taken as needed and scheduled.

 
   
Each photo I.D. badge for OU-HCOM students, interns, residents, and visiting students will indicate the following:
  • Ohio University Heritage College of Osteopathic Medicine
  • College Logo
  • Year of Medical Education
  • Center for Osteopathic Regional Education (CORE), and associated hospital, if Year III or Year IV
  • Level of Postgraduate training, if applicable
  • Affiliated Hospital if Intern/Resident
  • Expiration Date

If a student's personal appearance changes drastically between the third and fourth year, a new photograph will be taken only upon the student's request to be used on the Year IV I.D. badge.
 

   
Policy

The following requirements relative to the OU-HCOM Photo I.D. badge must be adhered to by each OU-COM student/post-graduate physician:
  1. The OU-HCOM photo I.D. badge will not supersede the Ohio University I.D. card issued for university services.
     
  2. The OU-HCOM photo I.D. badge must be visibly worn at all times (preferably on the lapel of the white consultation jacket) while in the clinical setting. This includes the simulated patient area and in lab situations as required by course coordinators.
     
  3. The OU-HCOM photo I.D. badge must be visibly worn at all times while taking an OU-HCOM exam, unless informed otherwise by the course coordinator.
     
  4. The OU-HCOM photo I.D. badge must be presented upon request while using the Heritage College of Osteopathic Medicine facilities. Access may be denied to facilities such as the Learning Resource Center, etc., if a valid I.D. badge is not presented. When in the Learning Resource Center, you will present your OU-HCOM photo I.D. badge for identification purposes and for checking out material.
     
  5. The Year III photo I.D. badge must be returned to the student's Center for Osteopathic Regional Education (CORE) office.
     
  6. A lost, stolen, or damaged OU-HCOM photo I.D. badge must be reported to a) the Office of Student Affairs by Year I and II students, b) Academic Affairs by Year III and Year IV students, c) Southeast CORE office by visiting students, and d) Director of Medical Education office for interns and residents. Arrangements must be made by the individual for another photo I.D. badge to be processed. Privileges associated with this I.D. badge may be denied if arrangements to have a duplicate badge issued are not made promptly.
     
  7. If a student/post-graduate physician changes his/her last name and has submitted required name change information to the Office of Student Affairs, a new OU-HCOM photo I.D. badge must be issued by the responsible unit.
     
  8. A replacement fee may be charged to have a new OU-HCOM photo I.D. badge made, regardless of the reason.
     
  9. Year III and Year IV students who are required to wear a hospital I.D. card at their CORE, must also visibly wear the OU-HCOM photo I.D. badge.
     
  10. The OU-HCOM photo I.D. badge should not be defaced in any way.
   
Procedures

Year I, II, and III students will be notified of the time and place that pictures for OU-HCOM photo I.D. badges will be taken by the College's Photographic Services. It is the student's responsibility to be available, or to notify the Office of Student Affairs or Academic Affairs of any conflicts. Students are to wear appropriate attire when sitting for photos. For Year III and IV students, white consultation jackets (lab coats) are required. Males are required to wear ties. Privileges associated with the use of the OU-HCOM photo I.D. badge may be denied until the badge is officially issued.

The OU-HCOM photo I.D. badges will be issued by the Office of Student Affairs according to established guidelines for Year I and II students at the time the pictures are taken. Year III students must personally sign for I.D. badges in Academic Affairs at the time they are issued. Fourth year photo I.D. badges will be issued at the end of year three (Spring Quarter) by the Administrators at the Centers for Osteopathic Regional Education (CORE).

Replacement of a lost, stolen or damaged OU-HCOM photo I.D. badge for Year III or Year IV students, while at their CORE location, will be arranged by the CORE Administrator and Regional Education Programs in Academic Affairs on the main campus.
