COMLEX Scores, and Dean's
Letters of Recommendation |

Before you graduate, the final
step of your formal education is applying for an
internship and/or residency. There are many items that hospitals request. While these
requirements may seem tedious, it is important to
"follow the procedures" and allow plenty of time so your internship application
process runs smoothly. Here is how to obtain these
forms: |

Transcripts:
The Office of Student
Affairs does not process transcript requests.
All transcript requests should be directed to
the
Office of the University Registrar.
|

|
|
The following information
regarding COMLEX scores and Dean's letter requests
is for NON ERAS applicants ONLY.
Most osteopathic and
allopathic residency programs require students
to apply using ERAS (Electronic Residency
Application Service). These applications must be
filed through the Office of Student Affairs. The
ERAS program changes from year to year, so when
you begin to consider residency programs ask
them what their application procedure entails.

COMLEX Scores (Levels 1 & 2
only):
The Office of Student
Affairs is glad to reply to all written requests
for copies of Levels 1 & 2 COMLEX scores, free
of charge. Level 3 scores are not available to
us.
In your request, please
include:
- Your printed name and
signature
- Social Security
Number
- Date of Birth
- Complete name/address
information to mail your requested scores

Dean's Letter of
Recommendation:
Dean's
Letter of Recommendation Request Form
Request forms are also available in Office of
Student Affairs. These requests must be typed or
clearly written and contain complete
addresses of all hospitals, as well as the
student's signature. The request must be made in
writing to the Office of Student Affairs at
least four weeks prior to the deadline.
Phone call requests are not accepted; all
requests must be in writing.
