Please note that the registration process is two-tiered: forms must be filed with Ohio University as well as with OU-HCOM Student Affairs/Student Government Association.

Tier 1- Registration with Ohio University

Complete by June 15 annually.

All student organizations:

  1. Must register each year.
  2. Must comply with the Ohio University criteria for registration.
  3. Will be notified upon approval of the registration.
  4. Must register any fundraising event.

If you are forming a new organization, or if the organization was not registered last year, a copy of the organization’s constitution must accompany the registration form. If the organization has a national affiliation, you must also submit a copy of the national constitution or statement of purpose.

Tier 2- Registration with  OU-HCOM

The following guidelines are to be followed in order to be  recognized by OU-HCOM. All student organizations:

  1. Will complete the  OU-HCOM Student Organization Registration form below by May 29.
     
  2. Must elect new officers for the following academic year by April 1. The new and old President must attend a mandatory leadership meeting at a date and time to be announced. Treasurers attend a mandatory training meeting through OU Student Activity.  Advisors and other officers are encouraged to attend.
     
  3. Must have at least 10 active members in addition to officers. Active members attend at least fifty percent of all meetings and events. Drop a listing of active members in the Student Government mailbox before the end of the Fall Term .
     
  4. Must have a clinical faculty member as an advisor. Each organization is responsible for obtaining this advisor and for defining the advisor’s role beyond that described in the Advisor Section in How to at OU.
     
  5. Should have a mission or purpose statement consistent with the college’s mission and goals. This statement should be included on the OU-HCOM Student Organization Registration Form.
     
  6. Should indicate the amount the organization will charge individual members for annual or bi-annual dues.
     
  7. Will register all business meeting times and locations using the online Student Event Registration form.
     
  8. Will schedule at least three (3) "events" per term, submitting the OU-HCOM Event Registration Form at least one week in advance of the anticipated event. Two of the three events may be meetings. The third "event" is any scheduled activity by an individual or organization with the exception of regularly scheduled business meetings. If a speaker is invited to a business meeting, the form must be completed.

What is the organization’s mission statement/purpose? [This information will be used in OU-HCOM publications describing student organizations]

What service does the organization provide to the community, the college and/or the osteopathic profession? Does the organization have a philanthropy?  If so, what is it?

Please outline a rough schedule of events the organization plans to participate in or to sponsor this academic year. Indicate any other student organizations jointly involved. Indicate how the organization intends to finance these events [fundraisers, etc.].

Please also submit the on line OU Application form to register by June 1. If you have already organized events for the Fall Term, please submit them through the online OU-HCOM Event Registration form and OU’s Application to Register a Fundraising Event.

I have read the OU-HCOM Student Organization Registration Guidelines for 2010-2011 and submit these forms in order to be recognized as a student organization by OU-HCOM and to be eligible for educational programming funds from the Student Government Association.
